Improving Patients’ Quality of Life With a Hospice Care Team
When diagnosed with a life-limiting illness and a cure is no longer possible, hospice may be a good option to support the patient’s care plan. Our hospice care team provides services in the patient’s residence, whether they live at home or in an assisted living facility. We pride ourselves in providing compassionate and excellent care that helps enhance the patient’s quality of life. Empathetic and respectful care can make a valuable difference in a patient’s experiences as the end stages of life progress.
Our Hospice Care Team Approach
Our team members act as an extension of your medical practice. A & H Hospice employs highly trained and certified healthcare professionals with hospice experience. Our hospice care team members include:
Registered nurses
Medical directors
Certified nursing assistants
Dietitians
Therapists
Social workers
Spiritual support professionals and bereavement specialists
Care coordinators
We work with patients and their families to create personalized care plans. Some of the services we provide include:
Pain and symptom management
Assistance with the daily tasks of living
Light housekeeping
Bereavement or spiritual counseling
In-patient, continuous, and respite care
Our hospice care team offers rapid response rates, aiming to provide service within 24 hours once contacted. Hospice qualifications for MediCare and MediCal patients offer zero out-of-pocket costs with A & H Hospice. We also accept many private insurances.
Determining the Need for Hospice Care
For a patient with a terminal diagnosis, hospice may be a good option with any of the following conditions:
Health continues to decline even with care
The patient isn’t eating or drinking well enough to maintain their weight
Weight loss of 10 percent or more in six months
Recurring infections and frequent hospitalization
Plus some other less common reasons
Because an exact diagnosis for end-of-life can be difficult to determine, MediCare explains hospice qualifications can be extended to patients for periods of time beyond six months, provided they are re-certified for hospice every six months.
