The care team at A & H Hospice, Inc. is dedicated to providing compassionate, personal care to your loved one during their last stages of life. Routine Care

This is when patients receive care in a private residence or nursing home and it’s the most common form of hospice care.

In-Patient Care

In-patient care is a good option to help control symptoms of pain that cannot be dealt with at home. They go to the IPU (in-patient unit) for around 2-7 days until they are discharged.

Continuous Care

Continuous care is an option for when going to the IPU is out of the question. A member of our team will be present at least 8 hours a day and on call 24/7.

Respite Care

Respite care is for when you are taking care of a loved one who can’t be left alone for long periods of time. A member of A & H Hospice will look after your loved one for a maximum of five days.

Who Do We Serve?

Our Southern California hospice services include routine in-home visits, in-patient care, continuous in-home care, and respite care. A & H Hospice works with MediCare, MediCal, and private insurance to provide patients with exceptional palliative and hospice care. Patients with MediCare and MediCal will never incur out-of-pocket costs for their care.

We offer four different types of palliative and hospice care in Southern California. We work with you to assess your loved one’s condition and create an individualized plan for care. Our team will be available every step of the way providing the care you need:

  • Routine Care: Patients are cared for in their own home, a private residence, or the nursing home in which they reside. This is the most common form of hospice care.

  • In-Patient Care: This type of hospice care is used for short periods to help control symptoms of pain and discomfort that cannot be managed at home. Inpatient Unit (IPU) stays usually last 2-7 days before the patient is discharged home.

  • Continuous Care: When medical crises arise, the IPU isn’t always an option. In these cases, continuous care may be the best course of action. During our continuous care, a member of our team will be present at least 8 hours a day and on call 24/7.

  • Respite Care: When family members are taking care of a loved one who can’t be left alone for long periods, our respite care offers a bit of relief. For a maximum of five days, our team members will look after your loved one.

FAQs

  • To be eligible for hospice, patients must be diagnosed with a terminal illness and given the prognosis of an estimated six months or less to live. If the patient lives longer than the estimated six months, they must be recertified by a physician that they are still in the end stages of life. Then they may continue to receive hospice care

  • When entering into hospice care, medications and treatments to cure or control an illness must cease. Medications to control symptoms of pain or other conditions may continue. So for example, a person receiving chemotherapy to treat cancer must stop treatments. However, if that person has high blood pressure and pain, they may continue receiving high blood pressure and pain medications.

  • When it comes to end-of-life planning, it’s better to discuss palliative and hospice care far in advance. While this subject is difficult to discuss among family members, it’s best to have transparency about one’s wishes. This helps to reduce stress when itis needed. Having these discussions in advance can help the terminally ill make educated decisions with the advice and input of family and friends.

  • Patients with a serious illness that have been given a prognosis of six months or less to live are eligible for hospice and palliative care. Thus, care can be provided for six months at a time. If they are still living after six months, a doctor can recertify their condition to confirm they are at the end stage of life. From there, the patient can re-enter hospice care for another six months.

  • If a patient that has a terminal illness was given the prognosis of an estimated six months or less to live, they are eligible for hospice care. If the prognosis date has passed and they are still living, they will require a doctor visit to be recertified and to indicate they are still at their end stage of life. After this process, their hospice care continues.

  • Typically, hospice care is paid for by Medicare. It is the largest source of hospice payments in the United States. This is because they pay for 100% of the care, and cover about 84% of all patients in hospice. They pay hospice care centers a daily rate for each patient that is enrolled in their care.

  • Because hospice care focuses on symptom treatment and prioritizing the patient’s comfort, any curative treatment or disease management is not included. Hospice care is ideal for one who no longer can or does not wish to receive treatments for an illness, such as chemotherapy. Medications that only treat symptoms are available once a patient goes into hospice.
